it is not clear if you mean the AGM minutes and the Chair Report become one and the same document. Normally the AGM minutes are ratified at the next AGM when they have been written and everyone has had a chance to read them and vote on them. The Chairs report is read and voted in at the contemporaneous AGM... so is the plan that the Chair report is presented at the AGM, then the AGM minutes are added, then another meeting is held to agree the chair report with AGM minutes? i.e. how will the voting be recorded, as this will not be known in advance?

AGREE - the report template just needs to be a set of headings as per company annual reports. It is easy to get too complicated.
